Registration Forms:
We value your time and want to make the most of your visit at Birth & Family Clinic. To streamline your check in process and ensure that your record is complete and accurate, we ask that you print out a set of forms, fill them out, and bring them to the appointment with you. If the appointment is for your child, please also bring his or her immunization record.
If you are a new patient, or if it has been more than 3 years since your last visit, please complete the following forms:
If you are an established patient, but have not been seen at Birth & Family Clinic in the past 12 months, or are coming for your annual physical exam, please complete these forms:

Release of Information Forms:
Since 2006 BFC has been using Electronic Medical Records (EMR) to give providers timely access to patient labs, radiology reports, and Swedish/Edmonds records. We continue to learn and develop new applications for the EMR to better serve our patients and are committed to maintain the privacy and security of the health information. If you have any questions about your information or its use, please refer to our Notice of Privacy Practices or call our Medical Records department.
