Since 2006 BFC has been using Electronic Medical Records (EMR) to give providers timely access to patient labs, radiology reports, and Swedish/Edmonds records. We continue to learn and develop new applications for the EMR to better serve our patients and are committed to maintain the privacy and security of the health information. Requests for Medical Records

Starting February 21, 2012 all of our records requests will be processed by IOD Incorporated. IOD Incorporated offers over 30 years of experience in secure handling and timely copying of sensitive information.

To request a copy of your medical records, please open and print the Medical Records Request (PDF) form. Once you have fully complete the form, you can return it in person, by fax, or by mail to the clinic. Please allow two weeks, excluding holidays, for processing.
